Presentation: Persistent bilateral leg pain (8/10 NRS) despite technically successful L3–S1 fusion at another centre. Unable to walk more than 50 metres.
Diagnosis: Failed Back Surgery Syndrome with central sensitisation. CT confirmed solid fusion - re-operation not indicated.
Treatment: Spinal cord stimulation trial (10 days) → 70% pain reduction → permanent SCS implant.
Presentation: Power 3/5 bilateral lower limbs. MRI: T10–T11 vertebral destruction, epidural abscess, cord compression.
Diagnosis: TB spondylitis - confirmed by CBNAAT from CT-guided biopsy. Pott's disease with neurological compromise.
Treatment: Posterior decompression + transpedicular debridement + instrumented fusion + ATT for 12 months.
